To help you get started, we’ve created an Educational Accommodations Letter. This resource is designed to simplify the process of communicating your child’s needs clearly and effectively.
You can print the letter and select the symptoms and accommodations that apply to your child, adding any additional details as needed. From there, you may choose to bring the letter to your child’s physician to review and sign before submitting it to your school’s principal or teacher in preparation for a 504 meeting.
